On event request forms, every field following event description is squished into 1/2 of the screen. It still works, but it looks bad. Could these questions extend across the screen like the first few fields?

Also, I'd like all the questions to have some padding around them. Right now they bump up to each other and it is a little more difficult to read.

One more thing--when you add a "note" question type, the formatting is different than other questions. There is a large padding/margin beneath the note but not on any other question. This makes it look like the questions following the note are in a different category, but in most cases the note is giving context to the questions around it.
